The old adage that too many cooks in the kitchen spoil the broth works well here and substantiates the need for the medical home model.

Coordination of care across the continuum……a single physician overseeing a team to take care of chronically ill patient….now we’re cookin’ with gas.

When it comes to patient care, a single leader/coordinator is certainly a welcomed alternative to the chaos of multiple physicians making decisions in his/her silo.

Can you give us a real world example of where this advanced technology/semantic interoperability could be put to use to relieve some of the financial burdens or health problems of patients with poorly controlled diseases?
